Repeat Abortions: 5 Underlying (and overlooked) Factors Everyone Should Know

After an abortion, ovulation can return within one month if no birth control is used, and about 37.5% of women experience a repeat pregnancy within two years.

For some women, an abortion decision isn’t a one-time event but a moment that returns under the same or different circumstances. Examining the questions women ask, the associated safety and risks, and the statistics behind repeat abortions reveals the realities they face. What is a repeat abortion?

A repeat abortion is defined as having more than one induced abortion. An induced abortion is a procedure to end a pregnancy, chemically or surgically.

How did I get pregnant again so fast after my last abortion?

After an abortion, ovulation can return within one month if no birth control is used, and about 37.5% of women experience a repeat pregnancy within two years.

A desire for a “replacement” pregnancy – common after natural pregnancy loss – may also arise after abortion, especially when women feel pressured, strongly attached, or conflicted about the decision.

Surveys show that 29% of women seeking post-abortion counseling reported pursuing a replacement pregnancy. Another study of metropolitan teens found 74.9% became pregnant again within 24 months after their first abortion.

Is it safe to have another abortion?

Reports are often mixed when it comes to the mental health of women long-term due to most research not differentiating between chemical and surgical abortions when reporting outcomes. However, one study found women with a history of repeat abortions were at an increased risk of experiencing mental health issues after menopause.

Additionally, the National Library of Medicine conducted a study on following pregnancies and births of female mice who had been given the chemical abortion twice.

The results showed the female mice that had a repeat abortion, specifically repeated medical abortions, showed problems with:

  • Fertility
  • Miscarriage
  • Problems in later pregnancies: placenta issues, blood clotting difficulties, and disrupted hormones and metabolism.
  • Effects on offspring: reduced metabolism and lower sperm motility in male mice babies.

A large nationwide study found that even women who had one surgical abortion were more likely to experience complications in later pregnancies. These included preterm birth, weak cervix needing treatment, problems with the placenta, and heavy bleeding after delivery. Overall, this study found that women with a history of abortion had higher rates of these issues compared to women without previous abortions.

Why do I feel more conflicted this time?

Even when life circumstances seem similar, your feelings can shift over time. If you’re considering a repeat abortion, you might feel more conflicted now because:

  • You’re still healing emotionally from the previous abortion
  • You feel more attached to this pregnancy
  • Hormones are affecting you differently this time
  • Your situation has changed: different partner, finances, career, age, timing, or you’ve now had children
  • Your values or beliefs have shifted
  • Your mental health has changed, such as experiencing more anxiety or depression

If you’re feeling conflicted, give yourself time to sort through your emotions and explore all your options – not just abortion. A decision you made in the past doesn’t have to be the decision you make now.

Is something “wrong” with me for being here again?

Nothing is “wrong” with you.

Finding yourself in this situation again doesn’t mean you’re broken, less worthy, or incapable. You’re human, and life can be complicated. Contraception can fail, cycles can be miscalculated and sometimes life circumstances simply haven’t shifted in the way we hoped they would.

Research shows that approximately 60% of women who experience another unintended pregnancy after an abortion choose another abortion, most commonly because their financial, emotional, or relationship circumstances remain similar to those they were facing at the time of their earlier abortion decision.

While this may lead some to a repeat abortion, the deeper need is often not simply another procedure, but meaningful change in the circumstances that led to the pregnancy. This may involve addressing underlying factors such as:

  • relationship patterns or situations
  • access to effective birth control
  • abstinence for a given time
  • emotional health
  • seeking therapy for unresolved trauma (i.e. previous abortion, partner violence, sexual or childhood abuse)
